The CrisisOnCall Identification Cycle

 

Personal information about members are kept and made available to emergency staff during crisis situations. Only applicable information relative to the situation or crisis will be provided. Members must declare that information may be provided to emergency staff. Members must provide the information they want to be available to emergency staff.

 Accuracy and correctness of information is the responsibility of the member.

Members have a choice between the elastic or woven material armband for identification purposes. Each armband has the emergency call centre number on the outside and an unique member number on the inside. This enables emergency services staff to get hold of applicable information about the member from the call centre and helps them in deciding how best to treat the patient and relatives.

 

The following lifesaving information is available:

  • Medical aid fund information (if the member has medical aid cover)
  • Allergies
  • Chronic medical conditions
  • Chronic medication
  • Next of kin

Important Information

Personal information about the member and his/her dependents can be accessed by medical emergency staff during an emergency.
The following information are available:

  • Medical aid fund information (if the member has medical aid cover)
  • Allergies
  • Chronic medical conditions
  • Chronic medication
  • Other important medical information
  • Two selected next of kin living at a different address
  • Home address
  • Work address
  • Contact telephone numbers (home, work, cell, and others as applicable)
  • Nearest police station
  • Nearest fire brigade
  • Security company / armed reaction unit
  • Vehicle information
  • Contact details of clergyman / reverend
  • Contact details of two selected neighbours

Information is limited to that provided by the member. Members can change or update their information by email to clients@crisisoncall.co.za or by fax to 012 335 1881.

ALL INFORMATION ABOUT MEMBERS ARE TREATED AS CONFIDENTIAL IN ACCORDANCE WITH RELEVANT LEGISLATION.
